HIFEM Stem Cell Therapy

Combined protocol pairing HIFEM pelvic floor sessions with Secret99+ topical nano gel for pelvic strength and intimate wellness.

What it is

HIFEM Stem Cell Therapy at Klinik Amisi is a combined protocol that pairs a standard HIFEM pelvic floor session with a topical application of Secret99+ nano gel as an intimate wellness step. It builds on our HIFEM Pelvic Floor Therapy treatment by adding a second, external step in the same visit.

It is important to be clear about what “stem cell” means in this treatment name. The phrase refers to Secret99+, a topical nano gel formulation that includes stem-cell-derived ingredients applied externally to the intimate area. This protocol does not involve injectable stem cells, exosome injections, or any implanted cellular therapy. If you have come across this treatment expecting medical stem-cell injections, please ask our doctor at consultation — we will explain exactly what is included and help you decide what is right for you.

The HIFEM device used is BTL Emsella, the same FDA-cleared pelvic floor chair used in our standard HIFEM service. Secret99+ is a non-hormonal nano gel manufactured by Secret Inc (South Korea), used in clinical settings as a topical intimate wellness product.

How it works

The HIFEM portion works the same way as our standard pelvic floor service. The Emsella chair generates a focused electromagnetic field that passes through clothing and skin to stimulate the motor neurons of the pelvic floor, triggering thousands of supramaximal contractions across a 30-minute session. Across a course of treatments, this aims to help retrain and strengthen the pelvic floor and support neuromuscular control — useful for women with stress urinary incontinence or post-natal pelvic weakness.

The Secret99+ step is added after the HIFEM session. The topical nano gel is applied externally to the intimate area as an intimate wellness step. It is non-invasive, non-hormonal and applied without needles, incisions or internal devices. The two steps together form the combined protocol patients book under the “HIFEM Stem Cell” name.

What to expect

Before your treatment

Your first visit begins with a consultation. We will review your symptoms, obstetric history, any previous pelvic surgery, current medications and any implanted devices, as well as any allergies or skin sensitivities relevant to the topical step. Please wear comfortable clothing without metal zips or studs over the pelvic area, and empty your bladder before the session. Tell us if you have an IUD, are or might be pregnant, or have any implanted electronic device.

During the session

You sit fully clothed on the Emsella chair for the HIFEM portion. The clinician adjusts the position so the electromagnetic field is centred on the pelvic floor and gradually increases the intensity to firm but tolerable contractions across 30 minutes. After the HIFEM portion, the clinician will guide you through the Secret99+ topical application step in private. The full visit typically runs 30 to 45 minutes.

After your treatment

There are no specific restrictions for most patients. You can return to work, exercise and daily activities. Your clinician may give specific guidance about intimacy or bathing after the topical step on the day of treatment.

Recovery and downtime

This is generally a no-downtime treatment. There is no incision, no sedation and no recovery period required. The most commonly reported sensation after the HIFEM portion is mild pelvic muscle soreness, similar to that which follows an intense workout, lasting up to a day or two. Topical applications can occasionally cause mild irritation or sensitivity in patients with known allergies — tell your clinician promptly if you notice anything unusual. Results vary between individuals and tend to build progressively over a course of sessions rather than appearing after a single visit.

Who is this for

This combined protocol is typically chosen by patients who would otherwise be candidates for our standard HIFEM service and who also want to include a topical intimate wellness step in the same visit. It is one of several pelvic and intimate wellness options offered at Klinik Amisi, and our doctor will help you decide whether the standard HIFEM service or this combined protocol is the better fit for your goals.

You may be a candidate if you:

  • Experience stress urinary incontinence or post-natal pelvic floor weakness
  • Are at least 3 months post-vaginal-delivery or post-C-section and have been cleared by your obstetrician
  • Are peri- or post-menopausal with pelvic floor and intimate wellness changes
  • Want a non-surgical, non-injectable approach to pelvic and intimate wellness
  • Have had standard HIFEM previously and want to add a topical wellness step

Considerations and contraindications

All standard HIFEM contraindications apply to this protocol. It is not suitable during pregnancy, and is not appropriate for patients with cardiac pacemakers, implanted defibrillators or other electronic implants, or where there is metal in or near the pelvic area. The presence of an IUD should be reviewed individually with our doctor. Other situations requiring caution include recent pelvic surgery, hernia in the treatment area, severe pelvic organ prolapse, active pelvic infection, malignancy, fever, bleeding disorders and pulmonary insufficiency. Wait at least 3 months after vaginal delivery or C-section and ensure you have been cleared by your obstetrician.

For the topical Secret99+ step, additional considerations include known allergies or sensitivities to skincare or intimate care ingredients, broken skin in the application area, and active intimate infection. Please tell your clinician about any allergies, skin conditions or recent intimate procedures before treatment. Our doctor will review your full history and confirm whether this combined protocol is right for you.

Frequently asked questions

Does this treatment contain injectable stem cells?
No. This is an important point. The 'stem cell' in the name refers to Secret99+, a topical nano gel formulation that includes stem-cell-derived ingredients applied externally. There are no injections of live stem cells, no exosome injections, and no surgical implantation of cells. If you are looking for medical stem-cell therapy, this protocol is not that. Our doctor will explain exactly what is and is not in the treatment during your consultation.
How is this different from regular HIFEM Pelvic Floor Therapy?
The HIFEM portion is the same — a 30-minute session on the pelvic floor chair, fully clothed, delivering supramaximal contractions. The difference is that this protocol adds a topical Secret99+ nano gel application as an intimate wellness step after the HIFEM session. Patients who want the pelvic floor benefit only can choose standard HIFEM instead.
What is Secret99+ exactly?
Secret99+ is a non-hormonal topical nano gel manufactured by Secret Inc (South Korea), used in clinical settings as an intimate wellness product. It is applied externally and is not an injectable, oral or implanted product.
Is the treatment painful or uncomfortable?
The HIFEM portion is non-invasive and you remain fully clothed throughout — most patients describe it as a strong, unusual contraction sensation, similar to very rapid Kegels, but not painful. The Secret99+ topical step is a non-invasive external application. Tell your clinician about any sensitivities or allergies before treatment.
How many sessions will I need?
A typical course follows the standard HIFEM protocol of 6 sessions, scheduled twice weekly over 2 to 3 weeks. Your doctor may recommend a longer course or periodic maintenance based on your assessment and goals.
When can I resume normal activities?
There is generally no downtime after a session. You can return to work, exercise and daily routines immediately. Your clinician may give specific guidance on intimacy and bathing after the topical step on treatment day.
Are there side effects I should know about?
The HIFEM portion is generally well tolerated, with mild pelvic muscle soreness sometimes reported for a day or two. Topical applications carry a small risk of irritation or sensitivity, particularly if you have known allergies to skincare ingredients, broken skin or active infection in the area. Tell your clinician about any allergies or skin conditions before treatment.
Who should not have this treatment?
All standard HIFEM contraindications apply — including pregnancy, cardiac pacemakers or defibrillators, electronic or metallic implants in the pelvic area, recent pelvic surgery, hernia in the treatment area, malignancy, fever and bleeding disorders. Topical considerations include known allergies to product ingredients, broken skin, or active intimate infection. Our doctor will review your full history before treatment.
